ElderCare Options provides advocacy and guidance to families of elderly persons. The service begins with an in-person visit in the home or care-giving facility. A registered nurse geriatric care manager meets the family and conducts a thorough assessment to determine the elder's needs and identify strengths and weaknesses to create a plan. Your care manager will work with physicians, insurance, and care providers on behalf of the family.
